Dear Sirs, 

Kindly refer to writing agreement dated the __________________, by which both of you were appointed arbitrators to adjudged the differences between Mr.________________________, S/o____________________________________________, R/o_____________________________, and myself. per the said agreement you were required to appoint an umpire for the of the said arbitration; 

Now, I hereby ask you, by agreement, in appointing an umpire as stated above within fifteen days after the receipt of this notice. Kindly take notice that in default of such appointment I shall make an application to the court for appointment of the said umpire.
